Niagara Falls Memorial primary care networks achieves prestigious patient-centered medical home certification

Niagara Falls Memorial Medical Center announced its primary care network has received the prestigious patient-centered medical home (PCMH) certification from the National Committee for Quality Assurance (NCQA) for its four primary care practices located at the Golisano Primary Care Center, Lewiston Primary Care, and the two practices located at the Summit Healthplex in Wheatfield.

“This certification reflects Niagara Falls Memorial’s unwavering commitment to excellence in patient care and highlights our continuous efforts to improve our services and practices,” said Joseph Ruffolo, NFMMC president and CEO. “The PCMH certification will help Niagara Falls Memorial primary care centers further enhance service delivery and patient engagement.”

The PCMH certification reflects a rigorous evaluation process that recognizes health care practices meeting stringent standards for patient-centered care. This achievement is a testament to the dedication and hard work of primary care physicians, nurse practitioners, physician assistants, and support staff who strive to deliver comprehensive, coordinated and accessible care to Memorial’s 13,000 primary care patients.

“The PCMH model emphasizes a collaborative approach to health care, where the patient is at the center of the care team,” said Dr. Simmanjeet Mangat, chief of primary care at NFMMC. “This model improves the quality of care, enhances patient satisfaction, and helps manage costs by fostering a strong, proactive relationship between patients and their health care providers.”

The PCMH certification process involved a comprehensive review of primary care operations, including care coordination, providing access to care, and integrating patient feedback into service improvements.
