HART receives $40,000 grant from The Margaret L. Wendt Foundation

The Home Assistance Referral Team has received a $40,000 grant from The Margaret L. Wendt Foundation to support The UBMD Geriatric Home Care Support Initiative.

UBMD Primary Care and HART have entered a unique partnership wherein HART will serve as the home care providers for the UBMD Primary Care patients. Upon recommendation from their UBMD Primary Care physician, families – and those in need of home care – will have the opportunity to meet with HART staff in person to discuss their specific situation and determine what type of home care is needed.

“We are looking forward to growing our collaborative relationship with HART by offering home care services through their organization,” said Kevin Hughs, M.D., medical director, UBMD Primary Care. “To have the opportunity to partner with HART and know that our patients have this choice for home care is extremely important to all of our physicians.”

“We are grateful to The Margaret L. Wendt Foundation for this grant that will help so many with their home care needs,” added Christine S. O’Hara, MS, HART executive director. “HART is honored to partner with UBMD Primary Care on this extremely important initiative.”

The Margaret L. Wendt Foundation is a private grant-making entity founded in 1956 and dedicated to charitable and public service purposes in the Western New York area.

UBMD Primary Care is one of 16 practices within UBMD Physicians’ Group, Western New York’s largest medical group and partner of Erie Community Medical Center (ECMC), Kaleida Health and Roswell Park Comprehensive Cancer Center. Their board-certified physicians are faculty in either the department of family medicine or department of internal medicine at the Jacobs School of Medicine and Biomedical Sciences at the University at Buffalo.
