Horizon Health Services and Niagara Falls Memorial team up to offer holistic health services

Horizon Health Services is partnering with Niagara Falls Memorial Medical Center to improve community access to holistic health and wellness services and address a community need.

This collaboration between Memorial and Horizon Health, Western New York's largest provider of outpatient mental health and substance abuse services, promotes a holistic care model that addresses the physical behavioral and emotional needs of patients at all stages of life and ensures coordinated care for at-risk patients.

The gateway to these services is the Niagara Falls Memorial Primary Care Center, located across the street from the hospital at 620 10th St., Suite 704. There, a clinical team led by three primary care physicians and a physician assistant provides high-quality primary care with an emphasis on patient comfort and convenience.

Unique to this medical office is a certified counselor who provides universal screening for behavioral health and dependency concerns. The counselor then coordinates the provision of any additional services that patients may need, including referrals to one of Horizon's 17 locations for further evaluation.

"A wealth of evidence, examples and models exist supporting and illustrating primary and behavioral health care integration as a means for delivering quality care and improving overall health outcomes," according to the federal Center for Integrated Health Solutions.

Working with Niagara Falls Memorial Medical Center, Horizon Health hopes to not only supply specialized services to those who need them, but continue providing ongoing education to foster awareness of the services they offer to the community.
