NFMMC graduates first class of care coordinators

Niagara Falls Memorial Medical Center has graduated its first class of care coordinators under a New York State Department of Health grant to prepare workforce candidates for the future of health care.
The new care coordinators are the first of 120 individuals who will receive education provided in partnership with Niagara County Community College by way of this two-year project.
Care coordination is an emerging health care field that promotes deliberately organizing patient care activities and sharing information among all who are involved with a patient's care to achieve greater safety and effectiveness. Program graduates will perform care coordination and related care transition functions at primary care centers, hospitals, health homes, behavioral health agencies and other community-based organizations.
