NFMMC Health Home joins BestSelf

Niagara Falls Memorial Medical Center Health Home will now serve as a care management agency under the BestSelf Health Home and Health Homes of Upstate New York (HHUNY).

NFMMC stated, “Much like Memorial’s Health Home, BestSelf offers a wide range of programs and services to meet the community’s needs. Along with HHUNY, a management services organization providing administrative support and governance to its affiliated Health Homes, the partnership will undoubtedly have a positive impact on the care and services provided.”

Vicky Wideman, LMSW, NFMMC’s director of adult and children health home, said, “I am excited to be joining the BestSelf Health Home and HHUNY. This cooperative relationship will mean increased enrollment, allowing for more focus on the needs of the Niagara County community.”

Elizabeth Woike-Ganga, LCSW-R, president and CEO of BestSelf Behavioral Health, said, “We are very pleased Niagara Falls Memorial Medical Center Health Home is joining our BestSelf Health Home. We look forward to working with Memorial and their network of care management agencies to ensure Niagara County members continue to have their health needs met. The partnership will allow us to expand the reach of services we can provide to the community.”

A Health Home is a service that provides comprehensive care management to Medicaid recipients to make certain their medical, mental health and substance use disorder needs are being met. Individuals who enroll in a Health Home are matched with a care manager who helps to make sure that all providers involved in a member’s care are working well together and sharing the information that is important in supporting all aspects of the member’s health. NFMMC said, “Health Home members are more likely to receive preventive care and less likely to overuse expensive emergency rooms or require hospital admission.”

The transition will begin January 2022.
