Elderwood partners with DaVita Kidney Care

In partnership with DaVita Kidney Care, Elderwood is now able to accept rehab patients who require peritoneal dialysis.
Peritoneal dialysis removes toxins and fluid from damaged or diseased kidneys. Special equipment enables patients to perform their own dialysis at home, rather than at a clinic. However, until recently, a hospitalized PD patient requiring residential physical/occupational rehab therapy was forced to stay in the hospital for lack of a local subacute setting that could accommodate their dialysis treatments.
Elderwood at Wheatfield, 2600 Niagara Falls Blvd., Wheatfield, and Elderwood at Hamburg, 5775 Maelou Drive, Hamburg, are now the only subacute rehab therapy programs in the area able to accept peritoneal dialysis patients. This means PD patients can be discharged from the hospital sooner to begin their physical rehabilitation.
In preparation for the addition of PD service, several Elderwood nurses completed intense training from DaVita to learn how to run the dialysis machine and monitor the complex care of dialysis patients.
DaVita chose to partner with Elderwood because of the high quality of rehab care offered. Their PD patients are screened at the hospital for acceptance into Elderwood's rehab program. DaVita provides the supplies and works with Elderwood staff to coordinate delivery of the patient's dialysis machine, so their room is equipped and ready when they arrive for therapy.
Being able to accept rehab patients requiring peritoneal dialysis adds another facet to the quality rehab Elderwood provides.
