Mangat promoted to medical director of Memorial primary care, health home services

Simmanjeet Mangat, M.D., has been promoted to medical director of primary care services at Niagara Falls Memorial Medical Center. In addition, she will serve as medical director for its health homes serving both adults and children.

Mangat has been a member of the medical center's primary care team since 2014, providing patient care at the Niagara Falls Memorial Primary Care Center.

In her new role, she will work with Memorial's network of primary care offices to ensure physicians and staff members adhere to patient-centered medical home principles and comply with its Level 3 accreditation standards. She also will work with hospital administrators to achieve the goals and objectives of the state's delivery system reform incentive payment program.

In her health home role, Mangat will facilitate and promote ongoing dialogue between adult and child health home care managers, providers and clinical support personnel with the goal of continually improving outcomes for health home enrollees of all ages. Those efforts will include connecting with community support agencies, utilizing best practices to refer patients to appropriate services, and monitoring the health status of those patients.

A graduate of Government Medical College in Patiala, India, Mangat has completed residencies in internal medicine and medical biochemistry and genetics. She is a member of the American College of Physicians.
