Niagara Health Home to host Friday celebration

The emphasis will be on wellness Friday, when the Niagara Falls Memorial Medical Center's health home holds a CHIN celebration from 3-6 p.m. in the parking lot of Refuge Temple, 15 Cottage St., at Heritage Court.

The all-ages event will feature a car show, prize raffles, refreshments and music by DJ Lasia.

CHIN stands for "Community Health Innovations for Niagara" - a plan that embraces the concepts of better health and better care at lower cost for Niagara County residents. The CHIN plan encompasses 28 different community health-oriented projects and programs being pursued by Memorial in partnership with more than 200 community-based agencies and organizations countywide. It includes strategies to integrate primary care, dental care, behavioral health and community services; link persons with intellectual and developmental disabilities to integrated mainstream health care services; increase adult health home enrollment; launch a children's health home; establish a mental health clinic for children; and link the frail elderly to health home services.

"A workgroup has been empaneled to develop ways to ensure that CHIN initiatives are coordinated and serve the maximum number of Niagara County residents," said health home outreach coordinator Charles Walker. "We are deeply rooted in the community and we invite the public to celebrate with us Friday and learn more about us."
