Kalos Health approved for expansion of MLTC services

Kalos Health, a managed long-term-care program providing care coordination for Medicaid recipients diagnosed with a chronic illness, has received approval from the New York State Department of Health to expand services from Erie and Niagara counties to Orleans, Genesee and Chautauqua counties.

Kalos Health provides individuals 18 and older with personalized care management services with the goal of helping members remain in their own home. Members work with care managers and a team of health care professionals to develop a comprehensive plan to ensure the member's health and wellness needs are met.

Since its inception in 2014, Kalos Health has grown to be one of the largest MLTC programs in Western New York.

Executive Director David Cascio said, "We look forward to working with those communities, enrollees, providers and stakeholders to improve the health of the individual and the community."

Expansion plans have been assisted by a NYSDOH "balanced incentive program" grant recently awarded to Kalos Health.

Kalos Health currently has offices in Niagara Falls, Lockport and at Erie County Medical Center, with plans for additional locations to support the expansion.
