Kalos Health applies for rural expansion of services

Kalos Health (formerly First Choice Health), a managed long-term care program providing care coordination for Medicaid recipients diagnosed with a chronic illness, has applied to the New York State Department of Health to expand services from Erie and Niagara counties to Orleans, Genesee and Chautauqua counties.

Kalos Health provides individuals 18 and older with personalized care management services with the goal of helping members remain in their own homes. Care managers work with the member and a team of health care professionals to develop a comprehensive plan to ensure the members' health and wellness needs are met.

Since its inception in 2014, Kalos Health has grown to be the largest managed long-term care program in Western New York.

President and CEO John Lomeo said, "A need for expansion into rural areas of Western New York was identified by the NYSDOH based upon the lack of service and providers in those areas."

Expansion plans have been assisted by a $200,000 NYSDOH balanced incentive program grant that was recently awarded to Kalos Health.
